| Medical Clinic
The providers in our medical clinic can address a broad range of medical services for any age, from birth to Elder. Our emphasis is on providing preventive, diagnostic, routine and ongoing health maintenance care. In addition, our providers can make arrangements for seeing specialists, if necessary, and/or hospitalization.
Services include: Laboratory, prescriptions (off-site), immunizations, health checks, family planning, OB/GYN-pre and postnatal external referral, diabetic program, podiatry, referrals, fitness center, transportation Program, dental referrals (off-site), and nutritional counseling.
The Medical Clinic also works to address our community's needs through the School Nurse Program at the Indian Community School, and the Choices Youth Prevention program.

DREAM of Diabetes care Program
Your diabetes team includes:
- Diabetes care coordinator/registered nurse
- Medical Providers, including a physician
- Referral/benefit specialist
- Community advocate
- Dietitian
- Fitness coordinator
- Podiatrist
- Social worker
Your
diabetes team depends on you to tell them how your diabetes core plan
is working and when you need their help. That is why YOU are the most
important member of the team.
Services we Provide:
- Diabetes-focused visits
- Diabetic education classes nutritional counseling
- Fitness center
- Laboratory services
- Counseling to help you cope emotionally with diabetes
- Podiatry (foot care)
- Help with insurance/
- Benefits. medical coverage
- Assistance
with referrals to specialty physicians, such as ophthalmologist (eye),
dentist, cardiologist (heart), & nephrologist (kidney)
Diabetes
is a chronic disease-it does not go away. however, you can live well
with diabetes. at GLIIHC, we have a team in place to help you manage
your diabetes.
At GLIIHC our responsibilities to you are:
- To treat you with respect, dignity, and consideration.
- To receive a prompt response to your needs and concerns.
- To keep all information in strict confidentiality.
For the best care possible, when participating in your own treatment program you should:
- Keep all appointments.
- In an emergency, cancel at least 24-hours before your appointment and/or as soon as possible.
- Provide
all necessary information such as: medical insurance cards, list of all
current medications, tribal enrollment information & picture ID
Mission
The
Gerald L. Ignace Indian health Center Diabetes Program mission is to
improve the physical, mental, spiritual, and emotional health of
Milwaukee's diabetic Indians. this includes diabetes prevention, early
diagnosis, and treatment of diabetes and pre-diabetes, prevention
complications and promoting education to medical providers, diabetic
clients, their families, and the community as a whole.
At GLIIHC we Believe that:
- A person with diabetes can live a long, healthy life.
- Diabetes can be well managed, preventing and/or delaying most complications.
- Diet, exercise, self management, and a team approach are key components in managing diabetes.
- Diabetes care must be tailored to meet the specific needs of each individual.
- Diet, exercise, and community awareness are key components in preventing diabetes.
- Education, behavior change, resources, and emotional support are needed to positively impact diet and exercise habits.
