2026 Vendor Application
Thank you for your interest in having a booth at the 2026 Honoring the Heartbeat Mental Health and Wellness Powwow. Please answer all questions on this form to be considered for a booth. Completed applications must be submitted no later than Friday, April 10, 2026.

NOTE: A completed application does NOT guarantee a booth space. All completed applications are reviewed by a committee and resource booths will be selected in a fair manner based on the information provided. Please be as detailed as possible when describing the mental health information and services the organization offers. Vendors will be notified via the email address provided if they receive a booth space.
There is no booth fee for community resource vendors. Booth spaces include an 8’ table and 2 chairs. Organizations should bring a tablecloth that fully covers the table provided.

The fee for a vendor space is $50.00. This includes a table space with an 8’ table and 2 chairs. Vendors should bring a tablecloth to fully cover the table provided. However, if a tablecloth is needed, one can be rented for a minimal fee of $5.00.
