New Patients
Each new patient needs to fill out an intake form and provide us with their insurance card and proof of ID.
For new patients to the behavioral health department, an intake form must be filled out before the first appointment is made, so that we can place you with the provider who is best suited to your needs.
New patients to the medical department and dental department should arrive 30 minutes before their first appointment to fill out an intake form.
